This funding opportunity is designed for applicants to propose how they will develop capacity for, deliver, and sustain evidence-based self-management education and support programs that address behavioral health and/or chronic pain among older adults and adults with disabilities.
Approximately $1,890,284 was available in total funding for 2020.
The following activities are not fundable:
Domestic public or private non-profit entities including state and local governments, Indian tribal governments and organizations (American Indian/Alaskan Native/Native American), faith-based organizations, community-based organizations, hospitals, and institutions of higher education.
Letters of Intent were to be submitted by December 18, 2020. Applications were to be submitted by February 2, 2021. Similar deadlines are anticipated, annually.
Approximately $2,000,000 is available in total funding for FY21. Up to eight awards will be granted. Awards range from $200,000 to $300,000. Project period is up to three years. Cost sharing/matching is not required.
